404 Error just sometimes

We have Apache/Tomcat3.2 running on Linux. Web apps using servlets/jsp works fine. Except one weird and annoying problem. From time to time when page or servlet is requested it goes straight to 404 Error, there is no track of request in httpd access file at all. I thought it might be caching problem we have all anti-caching stuff in headers plus random number is send as part of URL to avoid proxy caching. But the problem is still there. It happens just from time to time.

If anyone can help I would ( and all of my users ) would really appreciate that !

Have you tried downloading the newest versions of Tomcat/Apache? It could be a glitch in the software.
